Serious Illness Nurse Navigator
Carolina Caring is seeking a dedicated and compassionate Registered Nurse to support and educate patients, families, and caregivers on serious illness and end-of-life care options. This role involves fostering strong relationships between Carolina Caring and local healthcare providers to ensure patients receive the highest level of coordinated care.
The Serious Illness Nurse Navigator will:
Employee Type:
Full-Time, Monday-Friday 8:30am-5pm
Minimum Experience:
Graduate of an accredited school of nursing – Bachelor’s degree strongly preferred. Must maintain CPR certification.
Required Degree:
One to three years’ clinical experience required. Hospice or home health care experience preferred.
