Honoring our nation’s heroes with compassionate end-of-life care.

The We Are Veterans program proudly provides our community’s veterans with specialized veterans hospice care tailored to their unique needs. A joint effort of the National Hospice & Palliative Care Organization and the U.S. Department of Veterans Affairs, this care pathway honors veterans facing the final months of life. Services are offered wherever the patient is most comfortable, whether that be a hospital, assisted living facility, or personal residence.

Advanced Cardiac Program

Providing cutting-edge cardiac care wherever you call home.

Our Advanced Cardiac Care Program is one of the first specialized programs to be established in collaboration with the National Partnership for Healthcare and Hospice Innovation and the American Heart Association. This innovative care pathway aims to address the underutilization of palliative medicine services for those suffering from advanced heart failure by offering personalized, at-home care to manage symptoms, reduce emergency room visits and improve quality of life.

Support for COPD

Helping you breathe easy with specialized nursing care.

Overseen by a clinical team with extensive experience in respiratory medicine, our program for chronic obstructive pulmonary disease (COPD) care focuses on managing symptoms so you can live a richer, fuller life. As part of this program, specially trained nurses visit your home to monitor your condition and provide personalized care. Educational resources and support are also available for caregivers.

How We Help:

Compassionate Care for Dementia

Addressing the challenges of dementia with dignity.

Navigating the uncertainties of dementia can be stressful for both patients and their families. Luckily, our team is here to provide guidance as the disease progresses. This specialized care pathway includes regularly scheduled home care visits by nurses educated in dementia care. During these visits, our medical staff will monitor your loved one’s health status and educate your family on effective caregiving strategies.
