Carolina Caring is hiring a Nurse Practitioner for the House Calls and Palliative Medicine Program in Gaston and Cleveland Counties. This role involves traveling to patients’ homes to provide personalized care.

Minimum Experience:
A minimum of two years in a Primary Care setting. Gerontology experience or certification preferred. Experience with CMS Chronic Care Management Services preferred.
Required Degree:
Graduate of an accredited school of nursing with a Master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner. Current, unrestricted North Carolina Nurse Practitioner license. Must maintain CPR certification. Must have active DEA certificate.
